Changing doctors during pregnancy: Keeping Your Care Records Together

Moving home or choosing a different pregnancy care provider can mean transferring records while appointments are already underway. The practical aim is continuity: knowing who is responsible for your care, which results are available, and what happens next.
Confirm that the new service has accepted you
Contact the new provider before assuming your care has transferred. Ask whether a referral is required, when you will first be seen, and who to contact during the transition. If you already have appointments booked, ask both services how to handle them.
Keep a written note of the agreed arrangements. Do not cancel a scan or specialist review simply because you have started a transfer request. Check which appointment replaces it and whether its timing matters.
Request the records that affect the next decision
Useful information may include your maternity notes, estimated due date, blood results, screening reports, ultrasound findings, medicines, and specialist care plans. Your providers can explain how records are shared securely and what you need to bring yourself.
A short pregnancy medical history can bridge gaps, but it does not replace clinical records. Flag any result you were told needed follow up, even if you do not have the report.
Explain what matters to you
Tell the new team about communication preferences, access needs, previous difficult experiences, or concerns that prompted the change. You can ask for a private conversation. It is reasonable to want the new clinician to understand the context without repeating every detail at every visit.

Close the loop
After the transfer, check that the new team has received the important documents and that your next appointment is confirmed. Save its routine and urgent contact numbers. If you develop worrying symptoms during the move, seek care promptly; an incomplete record transfer is not a reason to wait.
