Pregnancy with uncertain dates: How Your Team Estimates Gestation

Not knowing exactly how far along you are is a common reason to have questions at the start of pregnancy. Irregular periods, recent contraception, breastfeeding, or simply an uncertain memory can make the last menstrual period difficult to use. Your care team can still help estimate gestation.
Share dates without trying to make them fit
Write down the first day of your last period if you know it, whether that bleeding was typical, and your usual cycle pattern. Mention when you first had a positive pregnancy test and any fertility treatment. Give ranges when that is all you have.
A test date or a day you had sex does not always identify the exact day conception happened. Avoid choosing a date just because an app requires one. Mark it as provisional and discuss it at your appointment.
Understand what an ultrasound adds
A dating scan uses measurements to estimate how far the pregnancy has developed. An early ultrasound is more accurate for dating than a scan later in pregnancy. The estimated due date may change after the dating scan. That adjustment does not, by itself, mean something is wrong.
If you conceived through IVF, give your team the clinic records. The date is established using the embryo age and transfer date, rather than replacing these known dates with a routine scan estimate.
The scan’s timing and what it can establish depend on the stage of pregnancy and your circumstances. Your team will explain whether another assessment is needed. The guide to calculating a pregnancy due date explains why a due date remains an estimate.
Ask which date your care team will use
Once your team agrees a working due date, ask where it is recorded and whether your appointment schedule changes. Use the same date in your notes so that different calendars do not create unnecessary confusion. Once a reliable due date is established, later changes should be unusual and explained and recorded by your team.

Arrange care even before dates are settled
Contact maternity services when you know you are pregnant. Explain the uncertainty instead of waiting for a more definite answer. Contact your maternity service urgently for bleeding or concerning pain rather than waiting for a routine dating scan. Severe abdominal pain, heavy bleeding, fainting or collapse need emergency help now. Bleeding with shoulder tip pain also needs emergency help. A calendar estimate cannot assess those symptoms.
